Registration information and forms have been emailed to those with accepted abstracts.

You may register by sending your completed registration form, along with your payment in US dollars to the following address:

Registration forms and payments are due July 30, 2010.

Presenter Disclosure Form: All presenters (platform and poster) must complete the speaker disclosure form, even if you do not plan to obtain CME credit.  We cannot process your registration without your signed disclosure form.

Photos: Each year we publish photos of attendees in the meeting abstract book directory.  If you are new to the David Smith Workshop, have never submitted a photo in the past, or want to change your photo from previous years, please send your photo to Sarah Ruuska (dwsinfo@uw.edu) by July 30th.

FELLOWS
Participation by fellows and post-doctorate students is encouraged. Fellows/post-docs are invited dependent on acceptance of an abstract of research supervised and coauthored by their sponsor participant. We are currently seeking funding to provide support for travel and registration for some fellows that, if realized, will be awarded on a competitive basis determined by abstract quality.
